تعد عملية التسجيل أحدى العمليات الأساسية فى الخدمة الاجتماعية التى ترتبط ارتباطاً وثيقاً بجميع العمليات الأخرى کالدراسة والتشخيص والعلاج، ويعتمد نجاح التدخل المهنى للاخصائى الاجتماعى وعمله بدرجة کبيرة على معرفته بطرق وأساليب التسجيل المستخدمة فى الخدمة الاجتماعية، هذا بالاضافة إلى قدرته ومهارته فى تسجيل وتدوين العمليات المهنية التى يقوم بها، والممارس المهنى لطريقة خدمة الفرد فى سبيل اتمام عملية المساعدة يعتمد على القيام بعض العمليات المهنية ومنها التسجيل، والذى يعد اثبات للمقابلات والخطوات المهنية التى يقوم بها الاخصائى فى العمل المستمر مع العميل، وتلعب المهارات الکتابية دوراً مهاً فى حياتنا المعاصرة، ويعتبر القصور فى هذه المهارة معوقاً خطيراً من معوقات الممارسة الفعالة فى خدمة الفرد، فلا يوجد موقف اشکالى لا تتضمن خطة التدخل الخاصه به العديد من الاتصالات الکتابية، وإن الرسائل والتقارير والسجلات ومعظم المتطلبات البيروقراطية عندما تستخدم بشکل فعال فإنها تصبح أدوات نافعة فى تحقيق أهداف خدمة الفرد، ويعد تقييم الأداء المهنى نظاماً يهتم بتحديد کيفية قيام الاخصائى الاجتماعى بأعماله على فترة محددة، فتقييم الأداء المهنى صمم لقياس إلى أى مدى وصل الاخصائى إلى تحقيق متطلبات وظيفته، ويجب أن يعتمد على واقع محدد يرتبط بالممارسة المهنية فى إطار المؤسسة، ومن أهم أساليب تقييم الأداء المهنى الرجوع إلى الملفات والسجلات وفقد تحددت القضية الرئيسية للبحث فى الوقوف على واقع الممارسة المهنية لمهارة التسجيل مع الحالات الفردية.
يهدف البحث إلى:
1-  الوقوف على معارف الممارس المهنى المرتبطة بمهارة التسجيل مع الحالات الفردية.
2-  تحديد تطبيقات الممارس المهنى المرتبطة بمهارة التسجيل مع الحالات الفردية.

يسعى البحث للاجابة على التساؤلات الآتية:

اعتمد البحث على مفهوم مهارة التسجيل، والحالة الفردية، وينتمي هذه البحث إلى نمط الدراسات الوصفية بهدف الوقوف على الممارسة المهنية لمهارة التسجيل مع الحالات الفردية فى مجالات الممارسة المهنية، اعتمادا على المنهج الوصفى. وقد اعتمدت الباحثة في البحث الميدانى على مقياس مهارة التسحيل مع الحالات الفردية، وقد تم تطبيق البحث على جميع الاخصائيين الاجتماعيين العاملين بمجالات الممارسة المهنية الثلاث بمدينة الفيوم(مدرسى، وطبى، ورعاية اجتماعية) وعددهم (171) مفردة فقط،؛ وذلک في الفترة من 12/1/ 2020م – 19/2/2020م، وقد توصل البحث الإجابة على تساؤلاته ووضع دليل استرشادى لکيفية تسجيل الحالات الفردية.

The professional performance evaluation is designed to measure the extent to which the specialist has achieved the requirements of his job, and it must be based on a specific reality related to professional practice within the framework of the institution, and one of the most important methods of professional performance evaluation Refer to files and records, and this has been identified as the main issue for research in finding out the reality of the professional practice of recording skill with individual cases.
The research aims to:
1- Knowing the professional practitioner's knowledge related to the skill of recording with individual cases.
2- Defining the professional practitioner applications related to the skill of recording with individual cases.
3- Determine the obstacles to applying the professional practitioner associated with the skill of recording with individual cases.
4- Setting proposals to activate the professional practice related to the skill of recording with individual cases.
The research seeks to answer the following questions:
1- What is the professional practitioner's knowledge related to the skill of recording with individual cases?
2- What are the applications of the practitioner of recording skill with individual cases?
3- What are the obstacles in applying the professional practitioner of recording skill with individual cases?
4- What are the proposals to activate the role of the professional practitioner of the skill of recording with individual cases?
5- What is the level of contrast between the professional practitioners in applying the skill of recording with individual cases according to (gender - age - educational level - experience - training courses - field of practice)
         The research relied on the concept of recording skill and the individual case, and This research is considered from the pattern of descriptive research with the aim of standing on the professional practice of recording skill with individual cases in the areas of professional practice, depending on the descriptive approach. The researcher has adopted in the field research on the scale of the skill of referral with individual cases, and the research has been applied to all social workers working in the three areas of professional practice in the city of Fayoum (teacher, medical, and social care) and they are (171) single only, and that is in the period from 12/1/2020 - 19/2/2020, and the research reached to answer his questions and set a guide for how to record individual cases.
